With registration, I attest and consent to the following:

I understand this is a group setting event with no privacy and personal health information will not be discussed. I agree to respect the intention for this to be a safe and relaxing experience for all and refrain from unnecessary conversation during the treatment.
I understand this acupuncture treatment is to promote wellness only and does not constitute assessment, diagnosis or treatment of specific health concerns. There will be an opportunity to ask questions and have general concerns addressed.
I have informed the practitioner if I am pregnant, or have any of the following heath issues: seizure disorder, blood disorder requiring blood thinning medications, known or suspected recent direct exposure to contagious illness that may put a group at risk.
I understand I will be receiving an acupuncture protocol using sterile, single-use filiform needles and Clean Needle Technique to minimize the rare chance of infection at puncture sites. I acknowledge that there are other uncommon, but possible risks and side-effects of acupuncture including, but not limited to:

mild, momentary sharp sensation with needle insertions or removal,
minor bleeding or bruising at acupuncture points,
light-headedness or very rare fainting,
temporary or transitory sensations such as mild dull aches, pressure, tingling, or awareness of different points/areas of the body - your energy is engaging with the needles and some people are more aware of such sensations than others,
releases of strong emotion that may surface as tears, memory recollections, temporary waves of anxiety, or increased muscle tension or twitches, etc.
minor skin irritation to optional essential oil application,
exposure to possible airborne contagions unavoidable in a general public setting.

Acupuncture is considered highly safe and "side-effects" are most commonly very mild, and even pleasant, including deep relaxation, relief of tension or pain, improved sleep and digestion, and deeper sense of well-being. I acknowledge that not all risks or side-effects can possibly be anticipated, and that the practitioner is using the best professional judgement and training to minimize any unpleasant experiences. If at any time there there is discomfort, the treatment can be stopped and needles removed. Communication is always key.
